Pursuant to Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (OJ EU L 119/1 of 4 May 2016), hereinafter referred to as “RODO”, BoneReg Sp. z o.o. informs you that:

1. The Administrator of your data is BoneReg Sp. z o.o. with its registered office at 1 Ignacego Mościckiego Street, 24-110 Puławy, Poland.
2. The Administrator has appointed a Data Protection Supervisor (DPO) to supervise the correctness of data processing. He/she can be contacted at the e-mail address: office@bonereg.com.
3. The Administrator shall process personal data in the scope of contact data: name, surname, e-mail address, telephone number.
4. Your personal data will be processed by the Administrator in order to establish contact and document the work of the BoneReg Sp. z o.o. office. – The basis for processing your personal data is Article 6(1)(a) RODO.
5. BoneReg Sp. z o.o. does not intend to transfer your data outside the European Economic Area.
6. You have the right to access the content of your personal data and the right to rectify them, the right to request deletion, restriction of processing, the right to data portability, the right to object to the processing of your data, the right to withdraw your consent (if granted) at any time without giving any reason, without affecting the legality of the processing performed on the basis of consent before its withdrawal.
7. Your personal data will not be disclosed to other entities (controllers), except to entities authorised by law.
8. Your personal data may be accessed by entities (processors) to whom
BoneReg Sp. z o.o. subcontracts the performance of activities that may involve the processing of personal data.
9. BoneReg Sp. z o.o. does not use automated decision-making in relation to you, including profiling of you.
10. Your provision of personal data is voluntary, however, failure to provide such data will prevent you from contacting the office of BoneReg Sp. z o.o. via the contact form.
11. Your personal data will be processed for the time necessary to document your contact with BoneReg Sp. z o.o. office.
12. You have the right to lodge a complaint to the supervisory authority – the President of the Office for the Protection of Personal Data, if you consider that the processing of your personal data violates the provisions of the RODO.T
